Home Health Registered Nurse – RN

The Jaisohn Home Health Care RN is responsible for planning, coordinating, and documenting skilled professional nursing care in compliance with state and federal home health regulations and agency policies and procedures. Jaisohn HHC serves primarily the Asian American population. Bilingual in Korean/English, Chinese/English, Cambodian/English, or Vietnamese/English is a Plus! The RN participates in weekend, holiday, and on-call rotations. In the Philadelphia, Montgomery, Bucks, and Delaware Counties of PA.
Responsibilities
- Conducts initial nursing assessment through use of the OASIS assessment tool and including physical and psychosocial aspects of the patient.
- Utilizes own special body of knowledge to provide leadership and guidance to the home health team in formulating and individualizing the plan of care to address the patient’s needs.
- Contributes to identifying the patients’ changing needs both active and potential needs which may affect the care plan.
- Consistently collaborates with the physician to plan and implement the medical and nursing plan of care.
- Demonstrates behavior appropriate to a role model and leader. Adheres to a code of conduct, conducive to Jaisohn Home Health Care policy.
- Communicates in writing in the patient chart and verbally with co-workers and supervisors to coordinate interventions and ensure continuity of care.
- Facilitates and participates in team meetings and contributes to patient care planning.
Requirements
- Graduate of an accredited nursing program, a Baccalaureate degree in nursing or related field, preferred. Current license as a Registered Nurse in the Commonwealth of Pennsylvania.
- Minimum two (2) years in clinical nursing, with at least one (1) year in home health or community health nursing preferred.
- Demonstrates excellent written and verbal communication skills.

Jaisohn Home Health Clinical Director
Jaisohn Home Health Care is a non-profit ACHC-accredited, licensed home health care agency providing skilled nursing services, rehabilitation therapies, and other supportive services to Philadelphia, Montgomery, Bucks, Chester, and Delaware Counties.
Job Summary
The clinical Director Oversees and directs all aspects of Jaisohn Home Health clinical operations, staff management, and regulatory compliance.
Job Scope
- Main responsibility is to lead the overall JHHC patient care program.
- Manage patient and personnel assignments and patient care and referrals.
- Develop and maintain cooperative working relationships with homecare agencies to improve the overall level of JHHC services.
- Ensure that patient needs are being assessed and develop individualized care plans.
- Utilizes data resources and coordinates team for scheduling and supervising staff relative to assignments and visits.
- Oversee clinical records and chart audits, and ensure medical records are current.
- Assist with recruiting and recommending the hiring of home health staff (Speech Therapist, Registered Nurse).
- Improving the efficiency of clinical staff and maintaining positive relationships.
Experience
- Minimum 3 years of experience in nursing with 2 years in home health or community health field experience is required.
- Minimum one year in a supervisory or management role position.
- Ability to communicate effectively with individuals from multi-ethnic backgrounds.
- Basic knowledge of computer skills & electronic documents.

- Good Knowledge of quality assurance processes and best practices in home healthcare
- Prior experience as a Registered Nurse is required
